MDT Information

On Arrival

Patient is found lying on her side on the living room floor. She is visibly uncomfortable and moves very cautiously, grimacing with movement. She states she was bending over to pick up a laundry basket when she felt a sudden "pop" and severe pain in her lower back.

Initial Impression and Primary Survey

ResponseAlert and oriented x 4, able to communicate clearly despite pain (AVPU=A).
AirwayClear and self-maintaining.
BreathingRate and effort normal. Symmetrical chest expansion.
CirculationSkin warm, pink, dry. Radial pulse strong and regular. Capillary refill < 2 seconds.
DisabilitySignificant pain limiting mobility. Gross neurological function appears intact in lower limbs initially (able to wiggle toes). GCS 15.
Exposure/EnvironmentIndoor home environment, safe. Patient wearing casual clothes.

History

AllergiesIbuprofen (causes stomach upset)
MedicationsOral contraceptive pill. Occasional Paracetamol for headaches.
Past Medical HistoryOccasional episodes of mild lower back ache after exercise, but nothing like this before. No history of major trauma, cancer, osteoporosis. Generally healthy.
Last Oral IntakeBreakfast about 3 hours ago.
Events PrecedingWas doing laundry, bent forward to lift a basket (not excessively heavy) and felt an immediate, sharp pain in the lower back, centrally located. Described as a "pop" or "snap". Pain was instantly severe (8-9/10) and she had difficulty straightening up. Managed to slowly lower herself to the floor. Pain is constant, sharp/aching, worse with any movement, especially trying to stand or twist. No radiation down legs initially reported.
